    Summary

    The VA Grant and Per Diem Program supports time-limited case management services to improve the retention of housing by Veterans who were previously homeless and are transitioning to permanent housing.


    In 2020, the VA accepted applications under the following models:

    • Bridge Housing - Bridge housing is intended to be a short-term stay in transitional housing for Veterans with pre-identified permanent housing destinations. The target population for this model are homeless Veterans who have been offered and have accepted a permanent housing intervention (e.g., Supportive Services for Veteran Families (SSVF), Department of Housing and Urban Development-VA Supportive Housing (HUD–VASH), Housing Coalition/ Continuum of Care (CoC)), but are not able to immediately enter the permanent housing. Other permanent housing may also be identified (e.g., purchase of a home, or an apartment lease).
    • Low Demand - Low-Demand housing uses a low-demand/harm reduction model to better accommodate chronically homeless Veterans, and Veterans who were unsuccessful in traditional treatment settings. Programming does not require sobriety or compliance with mental health treatment as a condition of admission or continued stay. Overall, demands are kept to a minimum; however, services are available as needed. The goal is to establish permanent housing in the community, while providing for the safety of staff and residents. The target population for this model are chronically homeless Veterans who suffer from mental-health or substance-use problems, or who struggle with maintaining sobriety; and Veterans with multiple treatment failures who may have never received treatment services, or may have been unsuccessful in traditional housing programs. These Veterans may have not yet fully committed to sobriety and treatment.
    • Hospital to Housing - Respite care is a medical model to address the housing and recuperative care needs of homeless Veterans who have been hospitalized. The target population for this model are homeless Veterans identified and evaluated in emergency departments and inpatient care settings for suitability for direct transfer to a designated GPD Program for transitional housing and supportive care.
    • Clinical Treatment - Clinically focused treatment provided in conjunction with services effective in helping homeless Veterans secure permanent housing and increase income through benefits and/or employment. The target population for this model are homeless Veterans with a specific diagnosis related to a substance-use disorder and/ or mental-health diagnosis; Veteran actively chooses to engage in clinical services.
    • Service-Intensive Transitional Housing - Provides transitional housing and robust services that facilitate individual stabilization, increased income and movement to permanent housing as rapidly as clinically appropriate. 
    • Service Centers - Provides services and information to engage and aid homeless Veterans obtain housing and services. The target population for this program are homeless Veterans who are seeking assistance with obtaining housing, employment, medical care, or benefits.

    Eligible applicants may request up to 40 beds per model type. A separate application is required for each model.


    NOTE: For those applicants that are successfully funded for multiple models, VA will allow, without a change of scope, a flex of beds between the applicant's models at the same facility. This flex will be up to five beds or 15% the total awarded bed limit per medical center, whichever is greater.

     

    History of Funding

    Up to $25,000,000 was available for an anticipated 100 awards in FY18.

    Additional Information

    No part of an award may be used to facilitate capital improvements or to purchase vans or real property. Applicants may not receive funding to replace funds provided by any Federal, state, or local Government agency or program to assist homeless persons.

    Eligible applicants are 501(c)(3) non-profit organizations, 501(c)(19) non-profit organizations, State governments, local governments, and Indian Tribal governments.

    Award Details

    Individual award amounts vary. Awards provided under this NOFA will be for a period of one (1) year, beginning on October 1, 2020, and ending on September 30, 2021. Cost sharing/matching is not required.


    For applications for transitional housing bed models: Applicants are encouraged to consider the need in their community for transitional housing models that are more focused (i.e., Bridge, Low Demand, Hospital-to-Housing and/or Clinical Treatment) over the transitional housing model that is more general (i.e., Service-Intensive). To that end, applicants may request up to 15 Service-Intensive beds per application. If more than 15 Service Intensive beds are requested within the same application, then at least 60 percent of the additional beds beyond 15 must be for a transitional housing bed model(s) other than Service Intensive.
