    Summary

    The Health Foundation for Western and Central New York is dedicated to improving the health and health care of the people and communities of western and central New York.

    Based in Buffalo, NY, with a second office in Syracuse, NY, the Health Foundation serves the following counties:  Allegany; Cattaraugus; Chautauqua; Erie; Genesee; Niagara; Orleans; Wyoming; Cayuga; Cortland; Herkimer; Madison; Oneida; Onondaga; Oswego; and Tompkins. 

    The Foundation has three focus areas: 
    1. Vulnerable Older Adults – The Foundation has long been focused on improving the quality of care for older adults in our communities. Recently is has begun to see that, as important as that work is, is needs to make a greater investment in deferring the consequences of frailty and postponing its onset. To do this, it needs to both increase the opportunities and supports for people to remain more independent and we must reduce the triggers of decline such as falls, poor transitions of care and ineffective management of multiple medications.
    2. Young Children Impacted by Poverty - Young children impacted by poverty are the Foundation's priority because they are one of the most vulnerable groups of people in the regions it serves. The concentration and culture of poverty impacts individual families, children and community institutions that serve them, such as schools and recreation programs. The Health Foundation is focused on children birth to five because of their particularly high vulnerability and because of the importance of the early learning experiences and significant developmental milestones to be achieved during these years.
    3. Growing Community Health Capacity - The Foundation recognizes that the health and health care of older adults and children impacted by poverty cannot improve without bolstering the communities within which they live and the health care systems which serve them. Programs funded through this focus area are designed to create a positive learning climate which fosters improved learning skills and capacity of individuals, and stronger, more effective and innovative organizations. They strive to increase and sustain collaborative activity, and foster communities and systems that embrace continuous learning,

  • Eligibility Details

    Nonprofit organizations in the Foundation's service area are eligible to apply for funding. Based in Buffalo, New York, the Foundation serves the following Communities: Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming, Cayuga, Cortland, Herkimer, Madison, Oneida, Onondaga, Oswego, and Tompkins. 

    Deadline Details

    The Foundation makes periodic requests for proposals as funding becomes available and also periodically considers unsolicited funding request from nonprofit organizations in the counties served.


    Requests have no specific annual deadline and interested organizations are advised to contact the Foundation or check its website for the most recently updated information regarding the next deadline.

    The foundation has awarded 517 grants to 227 organizations totaling $19,533,682 over the past five years. Grant amounts vary by program and range from $5,000 - $75,000
