    Summary

    The Tufts Health Plan Foundation is dedicated to improving the health and wellness of the communities the company serves throughout Massachusetts, New Hampshire, and Rhode Island. The Foundation invests in communities that promote healthy living across the life span, with an emphasis on healthy aging.


    The Foundation is currently accepting requests in the following three focus areas: Systems and Best Practices, Policy and Advocacy, and Collaboration and Community Engagement. The Momentum Fund provides smaller awards to support communities and organizations pursuing early-stage and innovative age- and dementia-friendly efforts.

    • Systems and Best Practices: The Tufts Health Plan Foundation supports the work of nonprofit organizations to improve, influence and ultimately create healthy communities and age-friendly cities and towns. The Foundation funds activities in Health and Wellness, Workforce Development, Purposeful Engagement, and Field and Capacity Building that will:
      • Improve or build systems to support healthy living with an emphasis on the systems that are serving older adults
      • Bring organizations together to collaboratively achieve broader impact and change
      • Scale efforts to address community needs
    • Policy and Advocacy: Engaging in policy and advocacy is essential to building healthy communities. As communities work on age-friendly ideas that are relevant to them, the Foundation will support efforts that are inclusive, collaborative, address quality of life, and improve conditions for all across the lifespan. One aging framework we use is that of the World Health Organization Age-Friendly Cities initiative. Policy and Advocacy initiatives we will consider recognize the essential components that make up community wellbeing. They include:
      • Built environment
      • Transportation
      • Housing
      • Social and civic participation and inclusion
      • Employment
      • Communication and information
      • Community support and health services
    • Proposals should include:
      • Community/civic engagement of diverse stakeholders in communities to mobilize and affect change
      • Participation among key stakeholders, including the voice of older adults, to support planning, policy and advocacy initiatives
      • Expanded access to programs and services as well as system change/improvement
    • Collaboration and Community Engagement: The Foundation supports community leaders in their work with multiple stakeholders toward a common community goal. Collaboration and Community Engagement grants support inclusive communities where older people are an asset. This work could include current or new activities that directly or indirectly lead to age-friendly communities and address barriers to success such as limited access to transportation or health care, ageist employment policies or other conditions that contribute to the social isolation of older people. The Foundation will consider the intersection between policy and practice as well as collaborative community work in the built environment, transportation, housing, social and civic participation and inclusion, employment, communication and information, and community support and health services.
    • Momentum Fund: The Foundation's Momentum Fund is a mini-grant program designed to build on our region's energy, experiences and insights to create cities and towns that are great places to grow up and grow old. Momentum Fund mini-grants support communities in Connecticut, Massachusetts, New Hampshire and Rhode Island promoting healthy aging.
     

    History of Funding

    Through our community investments, the Foundation has given more than $38 million to Connecticut, Massachusetts, New Hampshire and Rhode Island nonprofits that improve healthy living with an emphasis on older people.   • Eligibility Details

    The Tufts Health Plan Foundation funds projects serving communities in Massachusetts, New Hampshire, Rhode Island and, Connecticut.


    Deadline Details

    Systems and Best Practices: Letters of Intent are to be submitted by January 22, 2021. Applications are to be submitted by April 2, 2021.

    Policy and Advocacy: Letters of Intent are to be submitted by July 23, 2021. Applications are to be submitted by October 1, 2021.

    Collaboration and Community Engagement: Letters of Intent are to be submitted by January 22, 2021. Applications are to be submitted by April 2, 2021.

    Momentum Fund: Applications were to be submitted by September 11, 2020. A similar deadline is anticipated, annually.

    Award Details

    Award amounts vary based on scope and size of projects.

    • Momentum Fund: One-year mini-grants of up to $10,000 will be awarded.
